The name you print in the above registration form will be the identical way that it will appear in the AMS database and on the pdf AMS Promotional Flyer that AMS will supply to you*. For tracking purposes and in order to correctly identify your company as the lead source that is entitled to the Finders Fee, (as described below), it is very important that you enter your company name as you want it to be known. All leads that AMS receives shall be time coded; in the event that another company or person submits the identical lead as yours, the lead that arrived first to AMS shall be credited for the Finders Fee. A Finders Fee is in the amount of three hundred dollars ($300.00) for each lead that turns into a Regular AMS Customer. A Regular AMS Customer is a lead that has successfully connected AMS services to their company’s telephone system on-hold port, completed the one-month free trial promotional period that your company offered to them through AMS, signed an AMS one-year Service & Rental Agreement with the standard AMS monthly service pricing (no discounts), and has paid the opening charges as stated in their AMS Service & Rental Agreement. Any additional location(s) of any specific lead that connects AMS Services within thirty (30) days of the start date of their initial Agreement with AMS, shall entitle you to receive an additional twenty-five ($25.00) for each respective added location. After the expiration of the 30-day period from the start date of the AMS Agreement at the respective initial location, that lead is no longer eligible to you for a Finders Fee for any other future locations that may choose to connect AMS services. All checks, or Finders Fee drafts issued to you by AMS, shall be paid once a month with an attached accounting of eligible leads paid; a list of all leads that AMS has received from you, but have not yet fulfilled the status of payment as described above, will be available upon request. AMS reserves the right to change these rules and agreement at anytime without cause or notice. AMS respects your right to personal privacy; to that end, AMS highly regards your information to be proprietary for AMS’s internal marketing purposes exclusively. We stress that your information entered here and collected later will NEVER be shared or sold to other organizations for commercial or any other purposes.

*At no time does AMS allow Fax Broadcasting of its promotional material, and to send such unsolicited Faxes is strictly prohibited by this document and by all applicable Laws of Jurisdiction. AMS NEVER allows unsolicited Fax Broadcasts, or individual unsolicited Faxings. The sender of such unsolicited Faxes shall be liable and responsible for all legal fees and corresponding judgment awards.
